Instructions

1. Leave number column blank.
2. Signature of Legal name (example: William, not Will)
3. Print legal name
4. Year of birth
5. Date of signature
6. Precinct number- it’s found in the Geaux Vote App, or with the Secretary of State.
7. Address- best to stack number and street, with city, state, zip under it in that tiny box
8. Witness- can be anyone 18 years or older. You can witness for others signing and they for you.
9. Witness date- MUST match date of signature
